What Happened?
Florida’s attorney general sued Pfizer (PFE) and CEO Albert Bourla on Thursday, October 1, alleging the company misled consumers about its COVID-19 vaccine. The same day, Pfizer presented detailed Phase 2 data for tilrekimig, an eczema antibody it is testing against Dupixent in Phase 3.
Shares closed down 1.40% at $28.12 while the S&P 500 rose 0.19%, trimming about $2.3 billion from market value, though the move cannot be pinned on either event. The third-quarter report due November 3, per Pfizer’s investor relations materials, will say more about the stock than either headline.
Florida Targets a Franchise Pfizer Now Guides to $4 Billion
The complaint, filed in St. Lucie County circuit court under Florida’s Deceptive and Unfair Trade Practices Act, alleges Pfizer called the vaccine safe without disclosing risks such as heart inflammation and used unfair tactics to dominate the market. Florida seeks an injunction, 10,000 per violation (5,000 when seniors or people with disabilities were harmed), and the return of profits, citing more than $80 billion in vaccine sales.
The allegations are unproven, and Pfizer said they have no merit, calling its vaccine statements “accurate and science-based.” Kansas brought a similar case in 2024. A Texas suit was dismissed by a federal judge in December 2024 under the PREP Act, a federal liability shield for pandemic products; Texas appealed, and whether that defense reaches Florida’s state-court claims is not yet known.
The franchise itself is shrinking. Alexandre de Germay, Pfizer’s EVP and Chief International Commercial Officer, told a JPMorgan conference on September 22: “2024, we’re making $11 billion on COVID between the vaccines and the antivirals. 2025, we made about $6.7 billion. 2026, we guide for $4 billion.”
Pfizer’s second-quarter release puts 2026 Comirnaty and Paxlovid revenue at about $4 billion, roughly 6.5% of the about $61.5 billion guidance midpoint. Penalties are counted per violation, so the potential cost cannot be sized yet. The conduct Florida wants stopped, though, concerns a franchise Pfizer already expects to shrink.

Tilrekimig Faces Dupixent Head to Head
At the EADV Congress, monthly doses of 400 mg, 200 mg, and 50 mg produced week-16 EASI-75 responses in 58.5%, 61.0%, and 47.8% of biologic-naive adults, against 9.1% on placebo. EASI-75 means at least a 75% improvement in eczema severity scores. Pfizer reported no treatment-related serious adverse events in the stages presented.
Those results came against a placebo. Patients have been dosed in two Phase 3 eczema studies, one comparing tilrekimig directly with dupilumab, which Sanofi (SAN) and Regeneron (REGN) jointly develop as Dupixent. Pfizer already has an approved oral eczema drug, Cibinqo, so tilrekimig would extend an existing franchise rather than open a new one.
Pfizer trades at a forward price-to-earnings ratio of about 10 and Sanofi at about 8, both below AstraZeneca (AZN) at about 15, per TIKR’s Competitors data. Beyond eczema, de Germay said on September 22 that Pfizer has “31 Phase III clinical study happening right now.” He did not discuss tilrekimig.

2026 Estimates Rise While 2029 Forecasts Slip

Analysts have lifted 2026 revenue estimates from about $61.2 billion at the end of March to near the top of Pfizer’s $60.5 billion to $62.5 billion guidance, per TIKR. They trimmed 2029 and 2030 forecasts by about 2% in the third quarter. Adjusted EPS beat in Q2, but the GAAP loss shows how charges still weigh on reported results.

Conclusion
The figure to watch is growth excluding Comirnaty and Paxlovid, which ran at 5% operationally in the second quarter. If it holds at 5% or better with the ~$4 billion COVID outlook intact, the business story stays ahead of the legal one. A slip below that would leave the roughly 6% dividend yield carrying the stock while Florida’s case and tilrekimig’s head-to-head trial against Dupixent play out.
