Intuitive Surgical Has Lost 45% From Its High. J&J’s New Robot Is Only Part of Why

What Happened?

Intuitive Surgical (ISRG) spent two decades with no serious rival in soft-tissue surgical robotics. On July 22, Johnson & Johnson announced that the FDA granted De Novo authorization to its Ottava Robotic Surgical System, the first table-integrated soft tissue platform, cleared for ten general surgery procedures in the upper abdomen. J&J is launching commercially with select U.S. customers now.

The clearance landed on a stock that had already fallen for seven months. Intuitive beat every line of its second quarter on July 16, revenue of $2.89 billion against $2.83 billion expected and adjusted EPS of $2.80 against $2.50, then dropped 14.15% because management held its procedure guidance instead of raising it. Shares closed at $332.02 on July 23, about 45% below the 52-week high of $603.88. 

What Ottava Won, and Where It Points

De Novo authorization creates a new device category rather than clearing a copy of an existing one, which makes it a meaningful regulatory result. Ottava’s indications cover gastric bypass, gastrectomy, cholecystectomy, splenectomy, gastric sleeve, small bowel resection, appendectomy, lysis of adhesions, fundoplication, and hiatal hernia repair.

That list matters more than the clearance itself. Cholecystectomy and hernia repair are precisely the high-volume, cost-sensitive benign procedures Intuitive has spent the past year building a lower-priced strategy around. J&J did not enter at the complex reconstructive end of the market. It entered where Intuitive is trying to expand.

The scope has real limits. J&J’s inguinal hernia trial is still enrolling, and the authorization carries no urology indication, historically the deepest part of da Vinci’s installed base. This is also not the first competitive entry, since Medtronic’s Hugo cleared in 2025, and JPMorgan analyst Robbie Marcus wrote that physician checks still point to Intuitive as the favorite given its entrenched integration into hospital workflow and academic programs. A limited launch with select customers is a starting position, not a share.

Intuitive Built Its Defense Before Ottava Cleared

The strategy Intuitive is running against this was laid out on the July 16 call, six days before J&J’s announcement. It is not a reaction.

CEO Dave Rosa described a portfolio spanning a continuum from complex reconstructive work down to high-volume repeatable procedures. The instrument of that strategy is XiR, a lower-cost fourth-generation system aimed at cost-constrained buyers. CFO Jamie Samath sized it plainly: roughly 130 XiR systems in the field after about a year, around 50 in the U.S., concentrated in ambulatory surgery centers. Intuitive placed 27 systems at ASCs in the quarter, 20 of them XiR. Asked how large XiR becomes, Rosa said he expects it to be “a significant part of our placements over the coming quarters.”

The second lever is price. Beginning in the first half of 2027, Intuitive will extend the useful lives on a subset of EndoWrist instruments to cut customer cost per procedure in high-volume benign cases. Samath declined to quantify the revenue impact, promising numbers on the next earnings call. Travis Steed of Bank of America pressed with an estimate of a five-point total impact over a couple of years, which he framed as a little less than the seven points from the comparable 2020 program. Samath would not confirm it.

The Growth Slowdown That Triggered the Selling

U.S. da Vinci procedure growth came in at 12%, down from 14% in the first quarter, and that two-point step is what broke the stock. Samath split the cause rather than blaming policy alone: “There’s likely some combination thereof the impact from ACA, but also we’re just seeing a little bit of the law of large numbers as well.”

Half timing, half arithmetic. The timing half has a return path, and Rosa argued that deferred conditions typically progress and will ultimately require treatment. The arithmetic half does not reverse, and U.S. bariatric cases fell at a high single-digit rate as GLP-1 usage displaces surgical volume.

International is not slowing. Procedures outside the U.S. grew 20%, with Europe and Asia each up 20% and rest-of-world markets up 22%. Benign volumes internationally, just over a quarter of the OUS business, accelerated to 37%. Single-port procedures grew 61%, Ion grew 36%, and total revenue rose 19%, with recurring revenue at 85%. Reported non-GAAP gross margin reached 70%, though that included a $36 million pretax tariff refund; excluding it, the figure was 68.7%.

Twelve firms cut price targets within a day of the print, including JPMorgan to $450 from $550 and Wells Fargo to $487 from $654, pulling TIKR’s Street mean from $565.08 on June 30 to $490.47. Ratings barely moved: 16 Buys, 7 Outperforms, 9 Holds, 1 Underperform, and 1 Sell.

The mid case reaches around $524 on revenue growth of about 11% and net income margins near 32%. It assumes no multiple recovery, which is the right posture with a competitor newly cleared.

  • Revenue driver one: international procedure expansion, with benign volumes outside the U.S. up 37%
  • Revenue driver two: newer platforms, single-port up 61% and Ion up 36%
  • Margin driver: fixed overhead leverage and product cost reductions, supporting full-year gross margin guidance of 68% to 69%
  • Primary risk: the 2027 extended-use instrument program, an unquantified reduction in revenue per procedure

The high case assumes revenue growth around 12% with net income margins around 34%, which requires Ottava staying confined to a limited launch while deferred U.S. procedures return. The low case models around 7% annualized, the output if deceleration proves structural and extended use compresses instrument revenue harder than management expects.

Conclusion

October’s third-quarter call carries the number that decides this. Samath owes investors the quantification of the extended-use program, and Steed’s five points over two years are the usable threshold: at or under it, current 2027 estimates hold. Materially worse, and the Street’s models are still too high after a 45% decline from the high.

Watch U.S. procedure growth in the same print. Management attributed half the second-quarter deceleration to care that returns. Another two-point sequential drop retires that explanation, and Ottava will be selling into a market that Intuitive is no longer growing into.
