What Happened?

Acadia Pharmaceuticals stock (NASDAQ: ACAD) is facing a major test over whether remlifanserin still has a credible path forward in Alzheimer’s disease psychosis after its Phase 2 trial narrowly missed its main goal. Remlifanserin is being developed to treat hallucinations and delusions associated with Alzheimer’s disease psychosis, an indication with no FDA-approved therapy specifically for those symptoms. Competition is also advancing, with Bristol Myers Squibb evaluating COBENFY across three Phase 3 ADEPT studies in Alzheimer’s disease psychosis. ADEPT-2 is expected to enroll about 500 patients, compared with 326 patients in Acadia’s Phase 2 RADIANT analysis, giving investors a useful benchmark as remlifanserin advances toward Phase 3. ACAD stock fell sharply following the RADIANT results, finishing around $22 per share as investors reassessed the clinical risk around one of Acadia’s most important pipeline programs.

ACAD stock fell because the 60 mg dose of remlifanserin narrowly missed the RADIANT study’s primary endpoint, increasing uncertainty about whether its efficacy will be strong enough in Phase 3. The dose improved SAPS H+D scores by 12.6 points versus 10.4 points for placebo, producing an effect size of 0.26 and a p-value of 0.0603, just above the conventional 0.05 threshold for statistical significance. The key secondary measure of overall psychosis severity was more encouraging, with an effect size of 0.37 and nominal p-value of 0.0077, while Acadia reported a favorable safety and tolerability profile.

Management used this week’s special call to explain how Acadia is approaching Phase 3 after the primary-endpoint miss. An analysis using modestly higher baseline psychosis criteria would have retained about 80% of Phase 2 patients while increasing the primary-endpoint effect size from 0.26 to 0.33, giving management additional information as it considers refinements to the late-stage program. CEO Catherine Owen Adams called the results a “highly Phase III enabling set of data,” while Acadia plans to remove the weaker 30 mg arm, continue Phase 3 with the 60 mg dose, and present additional efficacy and safety findings at the CTAD conference in November.

Wall Street also reset expectations after the readout. BMO Capital lowered its price target to $34 while maintaining an Outperform rating and reduced its estimated probability of success for remlifanserin to 35% from 40%, while Citigroup lowered its target from $40 to $33 and TD Cowen reiterated its Buy rating and $37 target. The reactions capture the debate around ACAD after today’s selloff: the primary-endpoint miss increased clinical risk, but the stronger secondary result, favorable safety profile, and continued Phase 3 development give remlifanserin another opportunity to demonstrate a clearer treatment effect.

Is Acadia Pharmaceuticals Undervalued?

Continued DAYBUE adoption and NUPLAZID demand provide the clearest near-term revenue levers. Acadia raised its 2026 total revenue guidance to around $1.3 billion, with DAYBUE expected to contribute around $500 million and NUPLAZID around $800 million when the company’s guidance ranges are rounded.

The model’s 15% operating margin assumption also looks measured against current consensus estimates. TIKR analysts expect EBIT to increase from around $110 million in 2026 to around $320 million in 2028, while EBIT margins are projected to expand from roughly 3% to 20% over the same period. That places the model’s 15% margin assumption below the 2028 consensus estimate and reflects meaningful operating leverage as revenue grows faster than Acadia’s cost base.

Remlifanserin remains the largest potential swing factor beyond Acadia’s existing products because successful Phase 3 results could add another growth engine, while another disappointing readout would leave the valuation more dependent on DAYBUE and NUPLAZID. Bristol Myers Squibb’s competing COBENFY program adds another variable, with three Phase 3 studies in Alzheimer’s disease psychosis expected to produce results by the end of 2026. Those readouts could provide an important clinical benchmark for remlifanserin as Acadia advances its own Phase 3 program.

Using roughly 15% revenue growth, 15% operating margins, and a 26x exit P/E multiple, TIKR’s valuation model suggests Acadia stock could reach around $34 per share, implying roughly 54% upside from around $22 per share. Under these assumptions, ACAD appears undervalued, but realizing that upside will depend on continued DAYBUE and NUPLAZID growth, improving operating leverage, and stronger Phase 3 evidence that reduces the clinical uncertainty surrounding remlifanserin.
