AstraZeneca (AZN) said on Sunday, October 25, that its Claudin 18.2 antibody-drug conjugate sonesitatug vedotin (sone-ve) doubled median survival in stomach cancer patients who had already tried at least two treatments: 10.0 months versus 5.0 months on investigator’s choice of therapy.

The doubling applies only to that third-line-plus group. Across all 532 patients treated in second line or later, median survival rose from 6.4 to 10.6 months, and the risk of death fell 42%. Reuters reported the result beat analyst expectations, which centered on a hazard ratio of about 0.7 and a gain of at least two months.

The Wrinkle Is Progression-Free Survival

The trial’s other primary endpoint, progression-free survival judged by independent review, came in at a hazard ratio of 0.77 with a p-value of 0.053. That sits a hair above the 0.05 cutoff, so it counts as a miss. Median progression-free survival was 4.2 months on sone-ve versus 3.8 months on chemotherapy.

None of this is new. AstraZeneca disclosed the miss on July 27 alongside the survival win. What ESMO added is the size of it. Investigators’ own assessment, an exploratory analysis, was kinder: a hazard ratio of 0.59 and 4.4 months versus 3.0 months. AstraZeneca oncology R&D executive Susan Galbraith says the dataset is robust enough to take to regulators.

How Big Can Sone-Ve Get?

AstraZeneca has guided to peak annual sales of $3 billion to $5 billion. Against 2025 revenue of $58.74 billion, that’s 5% to 9%.

The Street’s 2026 estimate puts revenue at about $63 billion, up 8%. That’s roughly $4.6 billion of new revenue in a single year, about what sone-ve would add at peak.

That’s a real product, not a new AstraZeneca.

Takeda Got There First on Progression

Two days earlier, Takeda (TAK) reported that its rival Claudin 18.2 drug, arcotatug tavatecan, met both primary endpoints. Progression-free survival ran 7.1 months versus 3.1 (hazard ratio 0.45), and survival 10.2 months versus 6.9 (hazard ratio 0.57).

The comparison isn’t clean. Takeda’s trial enrolled 464 patients in China and Japan only, and required Claudin 18.2 on at least 75% of tumor cells. AstraZeneca’s was global and allowed 25% or more. Galbraith says that lower bar could double the eligible pool. Astellas (4503) is the other name here, since its Vyloy targets the same protein but only in high-expression tumors, per Reuters.

Two Ways to Read AstraZeneca Stock’s Sone-Ve Data

The bull read rests on survival. In the third-line-and-later group, 38.1% of patients were alive at 12 months on sone-ve versus 18.1% on investigator’s choice of therapy. Separately, the trial’s safety results showed fewer grade 3 or higher treatment-related adverse events: 36.4% versus 44.8%.

The bear read rests on the 0.053. HSBC analysts said, as Reuters reported, the miss makes sone-ve harder to compare with Takeda’s drug, and even the top of AstraZeneca’s range is under 9% of last year’s revenue.

The survival gain is hard to argue with, so I’d expect regulators to focus on how much a near-miss on progression should count against it. A filing is the next marker, and the test is how a regulator weighs a 0.053 against a survival hazard ratio of 0.47.
