What Happened?

Johnson & Johnson (JNJ) closed at $263.81 on October 23, up 27.5% in 2026, and added two pieces of drug news within two days. Britain approved its oral psoriasis pill Icotyde that Friday, and new ERLEADA prostate cancer data arrived at the ESMO Congress on Saturday, after markets had closed for the week.

Forward earnings estimates have fallen since June as J&J absorbed deal costs, yet shares trade at around 24 times forward earnings, up from around 16 times in September 2025. The new data has to carry that multiple into a 2027 that Chief Financial Officer Joe Wolk says includes $1.36 a share of dilution.

ERLEADA Data and an Icotyde Approval Back a Growth Plan Built Without Deals

At the ESMO Congress in Madrid on October 24, J&J reported that ERLEADA plus hormone therapy, given for six months before and after surgery, cut the risk of recurrence or death by 62% versus surgery alone in high-risk prostate cancer. The open-label substudy randomized 408 patients, and the regimen is not approved for this use. The main PROTEUS trial, presented in May, showed a 20% lower risk of metastasis or death versus hormone therapy alone.

A day earlier, Britain’s medicines regulator approved Icotyde, the once-daily pill J&J launched in the U.S. this year. Shares closed down 1.42% that day, a session when Merck (MRK) and Eli Lilly (LLY) also fell and reports said J&J’s orthopaedics talks with Smith & Nephew had collapsed. On the October 13 call, Chairman and CEO Joaquin Duato said more than 20,000 U.S. patients had started Icotyde through nearly 9,000 prescribers, though J&J has not disclosed its sales.

Tom Cavanaugh, Executive Vice President and Worldwide Chairman of Innovative Medicine, said bladder cancer drug INLEXZO topped $90 million in Q3, with new patient inserts up 45% from Q2. He put its current population at roughly 4,000 patients and pointed to broader study populations of about 15,000 and 50,000. Wolk said the goal of double-digit growth by the end of the decade relies on internal investment: “It does not assume any inorganic additions at this moment in time.”

Deal Dilution Cut Forward EPS as the Multiple Rose

TIKR’s NTM normalized EPS estimate fell from around $11.80 on June 28 to around $11.10 on October 23, while the share price rose from $254.66 to $263.81. The drop lines up with the dilution J&J built into its July guidance for the Firefly Bio acquisition, announced June 8, and the Sail Biomedicines collaboration.

Wolk said the two deals take $0.64 from 2026 adjusted EPS and $1.36 from 2027. Excluding them, he said 2026 adjusted EPS growth would approach 9%, versus 2.9% at the guided midpoint.

At around 24 times, J&J’s NTM P/E sits above Merck and Novartis (NOVN), both around 17 times, but below Eli Lilly at around 33 times on TIKR’s competitor screen. That premium rests on drugs, with Innovative Medicine up 7.9% operationally in Q3 and MedTech up 2.5%. The $5.5 billion ovarian talc settlement still needs participation from at least 95% of remaining claims, a threshold Wolk said J&J is trending to meet.

Johnson & Johnson Advanced Valuation Model (TIKR)

The TIKR model’s mid case projects around $342 by December 31, 2030, about 30% above $263.81 or roughly 6% a year. TIKR consensus, thin at 10 analysts for 2030 revenue, has sales growing about 7% a year from 2025 to 2030 and normalized net margin near 31% in 2030. Duato named DARZALEX, TREMFYA, Icotyde, RYBREVANT and INLEXZO as J&J’s drug growth drivers.

Faster label expansion for INLEXZO and Icotyde is the upside path, while a multiple drifting toward Merck’s 17 times is the downside. The Street’s mean target is around $279, with 11 Buys, 5 Outperforms, 6 Holds and 1 Sell. At about 6% a year, the mid case describes a fairly valued stock rather than a bargain, which leaves the load-up case resting on new evidence.

Conclusion

Two readouts come first: J&J expects INLEXZO bladder cancer data and Icotyde psoriatic arthritis data later this year. The bigger test is December 8, when J&J gives more detail on its 2027 outlook against TIKR consensus of around $11.60 in normalized 2027 EPS. Guidance at or above that level, with the $1.36 of dilution inside it, would back a multiple near 24 times, while a number well short would make that multiple hard to defend.
